Medical Records

If you need to request medical records from one of our offices, please be aware that it could take up to ten business days for you to receive your records. We will make every effort to fulfill all medical records requests as quickly as possible. Please call your physician’s Care Team to request a copy of your medical records or should you have any questions regarding your medical records.
